About These Templates

The MACBIS HCD Unification (HCDU) team created these templates to help HCD practitioners get started with their research work, but you are not required to use them. Always remember to save a copy of these templates for your personal use, instead of editing the original.

For more information about why these templates are recommended, and how they fit into the way our community conducts research, be sure to check out our Research Community Norms.

Planning Templates

Resources to help you set your research project up for success.

Research Plan

A template to plan and carry out your research, and to highlight the purpose, goals, questions, and methods for your research.

Research Context One Pager

Use this template to introduce your research to an audience, such as leadership or research participants. We recommend including background on how this project arose, its aims, plus anticipated deliverables and timelines for results. Be sure you have also done due diligence to look at previous research so that you are not replicating anything.

Dovetail team page template

Use this page as a template for creating a page about your team in Dovetail.

Dovetail project template

A template project plan and template insights outline for Dovetail.

Recruiting Templates

Resources to help you connect with users, get the participants you need, and take care of any pre and post study communications.

Recruitment Email Templates

An email outline for participant outreach, with versions for different types of testing

Screener survey template

A survey template in google forms that can be created and shared to gather basic information and availability about potential participants

Informed Consent One-Pager

All research should have the informed consent of participants. However, historically across MACBIS HCD teams, there hasn’t been a standard way of asking for consent. This form (or the below verbal script) can be used uniformly across research projects to ensure participants have adequate information to willingly take part in research.

Verbal Script for Informed Consent

a conversation script for collecting informed consent that can be inserted into moderation or facilitation guides for research studies.

Thank You Email

Although not mandatory, it’s a nice idea to send a thank you note 24-48 hours after each interview to the participant. Here is an example template you can adapt to do so.

Reporting Templates

Resources to help you effectively share out the results of your research with stakeholders.

MACBIS Research Report Template in Dovetail

Use this Dovetail insight as a template for creating a research report in Dovetail. The format should mirror that of the one-pager (linked below).

MACBIS Research Findings One-Pager Report in Figma

Make a copy of this template to use for one-pager reports to the Centers for Medicare and Medicaid Services (CMS).

MACBIS Research Report Template in Google Slides ****
